Follow the steps below and answer each of the questions. For each question, provide the
proper descriptions and explanations in at least 1-2 sentences.
1. Why is a PHR a valuable tool?

health record (or one for a family member). Consider the variety of health care
providers that you consult (or have consulted).

Gather all phone numbers and addresses for any healthcare providers
you have had consultations with.
b. Would you need to collect record copies? From whom? Of what?
You would need to collect copies of medical records, lab results,
prescriptions and all other health information.
c. How will you obtain these?
You can request these records directly from your healthcare providers.
Some providers may provide electronic copies. While others may
provide paper copies.
3. Describe the type of information you would want to put in your (or loved ones) PHR.
Your PHR should include:
• Personal information (name, birth date, blood type)
• Emergency contacts
• Health insurance information
• Current and past health conditions
• Medications and dosages
• Allergies
• Immunization records
• Family health history
4. Decide the medium you would likely use (paper, web-based, software based, etc.).
The medium you choose for your PHR can depend on your comfort level
